The Norton Prevention & Wellness team works with community partners to provide cardiovascular health screenings at community events and health fairs.

Based on the details of your event, the following screenings may be available to individuals age 18 and older. Certain guidelines apply.

  • Blood pressure: High blood pressure can lead to heart disease, kidney disease, and stroke.
  • Blood sugar: High blood sugar is a risk factor for diabetes, heart disease, and stroke.
  • Body mass index (BMI): A high BMI (weight/height ratio) is associated with many chronic diseases.
  • Additional screenings may be offered on a limited basis.

Complete the form to request cardiovascular screening services and provide details about your event. Select “Adult Prevention and Wellness” at the bottom of the form to request these services.
